Continuity of Care
Our practice has strategies and policies that encourage continuity of comprehensive care by facilitating:
- the sense of affiliation between the patient and the general practitioner ‘my doctor’
- consistency of care by various people involved in the patient’s care, and
- maintenance of information across healthcare events through documentation, handover and review.
