45+ Health Check

The 45+ Health check is available those between 45 and 49 years of age at risk of developing a chronic disease.

A health assessment can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.

Chronic disease risk factors:

A chronic disease or condition is one that has been or is likely to be present for at least six months, including (but not limited to) asthma, cancer, cardiovascular illness, diabetes mellitus, a mental health condition, arthritis or a musculoskeletal condition.

Risk factors that the GP may consider include but are not limited to: Lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol use; biomedical risk factors, such as high cholesterol, high blood pressure, impaired glucose metabolism or excess weight; and a family history of a chronic disease.
What does the Health Assessment Involve?

  • The health assessment involves a 20-minute consultation with the Practice Nurse followed by a 20-40-minute appointment with the Doctor.
  • The aim of the health assessment is to identify health issues and conditions that are potentially preventable or controlled through interventions to improve health and quality of life.
  • The health assessment will include information collection, making an overall assessment of the patient’s health, initiating interventions and referrals and providing advice and information including strategies to achieve lifestyle and behaviour changes.

How much does it cost? FREE, *fully covered by Medicare Bulk Billed, No out of pocked cost. Please ask reception regarding booking in for your 45+ health assessment.

The 75+ Health check

We are currently conducting health assessments In Practice or At Your Home (Home visit will enable our nurse to assess safety measures to prevent slips, trips and falls).

Home visits with the nurse are available followed by in practice appointment with a GP.

Why would your GP suggest a health assessment for you?

Your GP or Nurse may suggest a Health Assessment for you to assist you to maintain good health. The assessment allows your GP to take specific notice of areas which are hard to cover in a normal consultation.

A Health Assessment also provides you an opportunity to raise any concerns that you may have about your health. (E.g. sleeping or hearing difficulties, nutrition, etc.)

What does the Health Assessment Involve?

The aim of the health assessment is to identify health issues and conditions that are potentially preventable or controlled through interventions to improve health and quality of life.

The Health Assessment, done by your GP and/or Nurse may involve:

  • Information collection
  • Updating patient details
  • Arranging examinations and investigations if required
  • Have seen other doctors or health workers in the last 6 months
  • Medication assessment/review
  • Immunisation Status

Providing advice and information to the patient as needed in areas such as:

  • Living arrangements
  • Home help
  • Home safety
  • Social support
  • Mobility

Review and advice regarding:

  • Smoking?
  • Alcohol
  • Nutrition
  • Exercise
